Hopeful for those we support, rebellious in our approach, and collaborative by breaking down barriers, AccessHope is seeking an Senior Oncology Nurse Navigator to help us uniquely deploy the latest cancer knowledge to the places it's needed most. As an ideal candidate, you'll work independently and as part of a team, to collaborate with subject matter experts, physicians, nurse practitioners, clinical specialists, and leadership to ensure a concierge-level of care and assistance through a member's journey for validation of diagnosis and treatment. The nurse Navigator's #1 priority is to ensure all clinical information is consolidated and up to date to ensure providers have what they need to review and make recommendations in the most efficient manner. Navigating a member with the utmost empathy and compassion is key to the success of this role. Leads and supports other team members, projects, and initiatives.
Key responsibilities
  • Reviews member records and abstracts relevant oncology history to create a clinical summary of a member's diagnosis and treatment timeline by partnering with internal and external sources to request and gather medical records, lab reports, and other pertinent member records for review. Demonstrates excellent attention to details as the individual reviews member files, completes forms and records with accuracy, and ensures files and summaries are accurate to member details.
  • Creates a clinical narrative summary to include the onset, diagnostic findings, treatment regimen history, and recommendations in sequential order prior to member's onsite or virtual consultation. Completes a post visit summary after onsite or virtual consultation per AccessHope guidelines.
  • Utilizes EMR to review medical records, order entry, and schedule appointments.
  • Participates and facilitates in all virtual introductions, virtual consultations, and onsite consultations.
  • Collaborates with patient navigator to assist member with accessing cancer care and navigating healthcare systems. Assesses barriers to care and engages members and families in creating potential solutions. Identifies appropriate and credible resources responsive to member needs
  • Collaborates with client partners and management on a regular basis to ensure continuum of care and customer experience is world-class
  • Provides follow up communication with member as needed.
  • Leads and provides support to junior positions, projects, and initiatives as directed
  • Identifies more complex cases for specialty consultation needs.
  • May be utilized to support other areas.
  • Collaborates with teammates and utilizes the AccessHope on-line library
  • Practices a high level of integrity and honesty in maintaining confidentiality

Required qualifications
  • Bachelor's degree in Nursing
  • 5+ years of clinical oncology nursing experience
  • Current licensed CA Registered Nurse (RN)
Preferred qualifications
  • Master's in Nursing
  • Oncology Certified Nurse (OCN)
  • 1 year experience writing narratives in a clinical or research environment
