A normal family planning is a goal for young women with autoimmune rheumatic diseases (ARD) since the improvement in management has made it possible. Few data are available on the use of traditional and novel anti-rheumatic drugs during pregnancy and lactation. There are many obstacles in evalutating the safety of drugs: a) pregnant women are excluded froml trials for the approval of new drugs;b) it is difficult to perform high quality studies because of ethical issues, e.g. making pregnant patients accepting randomization for preventative treatments, especially placebo;c) there is still a radicated cultural reluctancy in taking drugs during pregnancy and lactation. Registries are best options to collect data from everyday clinical practice and yield valuble information for the counselling of patients wishing for a pregnancy. The Italian Registry of prospectively followed and treated pregnancies will capture this information for the first time at national level with adequate sample size.
